Preparing for Your Visit

The Ryan Center is located in the Center for Advanced Medicine on the University of Chicago Hospital campus. The address is 5758 S. Maryland Ave. The clinic is on the third floor in unit 3H. When you check in at 3H, let them know that you are there to see the Ryan Center.

What to Bring:

• Your ID card
• Medical card or insurance card (when applicable)
• Fee or co-pay. You will be informed about the fees when you make your appointment
• The Ryan Center accepts cash, money orders and credit cards.

Things to Remember:

• We don’t allow small children or babies to accompany you to your appointment. Please arrange for childcare.
• Parking can be very difficult, so please allow plenty of time to park. If you are more than 30 minutes late, the clinic may not be able to see you.
• If you will be receiving medication for sedation you MUST bring someone who will assist you in getting home after your appointment. That person must be present in the clinic before sedation can be administered.
• Please remember to call the Ryan Center at 773-834-9995 if you need to cancel or reschedule your appointment.
